NOTICE OF REQUEST FOR STATEMENT OF QUALIFICATIONS FOR ROOFING CONTRACTORS 1. Intent: The Parker County Hospital District is requesting statement of qualifications for roofing contractors capable of installing a structural standing seam roofing system. Interested roofing contractors are invited to submit a Statement of Qualifications (SOQ) no later than Tuesday, September 15, 2015 at 12:00 pm CST. For consideration, responders must submit one (1) electronic SOQs to randall.young1@pchdtx. org. The SOQs will be evaluated and the group of potential bidders shall be reduced to a group of qualified Contractors. The District expects to award a contract to the proposer who submits the proposal that, in the Districts estimation, represents the best value to the District. The Owner reserves the right to select or reject any/all proposal(s) in their best interest. The Owner also reserves the right to reject any/all proposers as unqualified, including without limitation the right to reject any or all nonconforming, nonresponsive, unbalanced, or conditional, qualifications. The Owner reserves the right to re-solicit, waive all informalities not involving price, time, or changes in the work to negotiate contract terms with the apparent successful proposer. Budget: The estimated roofing cost is TBD. 2. Description of Work: The successful contractor shall be responsible for all aspects of work, including but not limited to approval, permit, construction and warranty of a Structural Standing Seam Roofing System. 3. Statement of Qualifications: SOQs should include and will be evaluated upon the following: • Company information including: contact person, time in business, full-time employees. • Similar project experience in the past 7 years including: projects for other public agencies, self -performed work capability, and safety and quality program. • Evidence of 3 projects of similar size and scope completed in the past 7 years including: location, summary of scope, list of self performed work, and current owner references. • Statement of bonding capacity and insurance. 4. Provisions: The District reserves the right to act in its best interest and may terminate, modify or suspend the process, reject any or all submittals, modify the terms and conditions of this selection process and/or waive any informalities of any submission. 5.
