Mason Lewis Adoption IN THE CIRCUIT COURT OF DECATUR COUNTY STATE OF INDIANA CAUSE NO. 16C01-1804-AD-000004 IN RE: THE MATTER OF THE ADOPTION OF: MASON R. LEWIS NOTICE TO MOTHER Melissa Fay Carpenter is notified that a petition for adoption of Mason R. Lewis, DOB: June 30, 2008, was filed in the office of the Clerk of Decatur Circuit Court, 150 Courthouse Square, Greensburg, IN 47240. The petition for adoption alleges that the consent to adoption of Mason R. Lewis is not required because Melissa Fay Carpenter has failed to maintain a relationship with the child and has not supported the child when required by law to do so. If Melissa Fay Carpenter seeks to contest the adoption of Mason R. Lewis, she must file a motion to contest the adoption in accordance with I.C. 31-19-10-1 in the above named court no later than thirty (30) days after the date of service of this notice. If Melissa Fay Carpenter does not file a motion to contest the adoption within thirty (30) days after service of this notice, the above named court will hear and determine the petition for adoption. The consent to adoption of Mason R. Lewis will be irrevocably implied and she will lose the right to contest either the adoption or the validity of her implied consent to the adoption. No oral statement made to Melissa Fay Carpenter relieves her of her obligations under this notice. This notice complies with I.C. 31-19-4-5-3 but does not exhaustively set forth a person's legal obligations under the Indiana adoption statutes.
