Public Notice HMH Riverside Clinic As of Tuesday, August 21, 2018, the HMH Riverside Clinic, located at 3638 Highway 19, in Huntsville, Texas, will permanently close. Providers and staff will relocate to the HMH Huntsville Clinic, at 125 Medical Park Lane, Suite C, Huntsville, Texas 77340. The Huntsville Clinic is open Monday thru Friday 8 am to 8 pm, and 9 am to 4 pm each Saturday. Letters are being mailed to patients that include resources for clinic transition and obtaining medical records. Records can be requested with a written release of information by contacting 936-291-3219.
