We canʼt fully express our deep gratitude for your loyalty and support. Working with you has been a great pleasure. Our last effective day of business will be August 22,2018. Until that date, we will continue to service our patients as normal. If you would like any changes to your account before August 22, 2018 please contact us at 606-864-0510 or office@cvplondon.com. We ask that, regardless of any account changes, you pay all outstanding invoices in full. We are pleased to announce that starting September 2018, Dr. Prathiba Ram M.D. will be transitioning to Rural Mental Health Solutions, 201 West Dixie Street, London, KY 40741 where she will continue to provide Psychiatric services. Patients can call 606-770-5170 to schedule an appointment or physician referrals can be faxed to 606-770-5171. Please note that all Cumberland Valley Psychiatry patients will be transferred to Rural Mental Health Solutions on August 24, 2018, unless otherwise requested. If you would like your records or account information transferred to another provider, please notify CVP by August 22, 2018 with your request. Patientʼs records will be available for transfer after August 22, 2018, requests should be submitted to Rural Mental Health Solutions by fax only 606-770-5171.
