Said application alleges that you consent to this adoption is not required by law for the following reasons, to-wit: 1. Consent is not required pursuant to OKLA. STAT. ANN. tit. 10 §7505-4.2 Since the biological Mother, the father listed on the birth certificate, and Putative Father have failed to pay child support 12 of the 14 months. 2. Biological Mother, the father listed on the birth certificate, and Putative Father have failed to exercise reasonable visitation rights. 3. Failure to appear at the hearing on said Application to Proceed Without Consent, shall constitute a denial of interest in the child which denial may, without further notice of this or any subsequent proceeding, in granting the adoption. You are further notified that this Court will hear the evidence in support of and in opposition to the granting of said Application and said petition on January 15, 2016 at the hour of 9:00 o’ clock a.m. at the Courthouse of said District Court of Woodward, in the Courthouse, at Woodward, Oklahoma. If you have any cause to show why the above mentioned Application and Petition should not be granted by the Court or why said child is not eligible for adoption without your consent, you should appear and present the same at the above stated time and place.
